University students (TriGen@Northwest) or Healthcare Professionals (TriGen@SGH) team up with youth volunteers to address the medical and social needs of seniors through monthly home visits. This programme also offers youths the chance to be health advocates for seniors within the community. Homecare is an intergenerational program committed to delivering comprehensive care for seniors.

TTRIGEN@NorthWest Timeline

1st Phase

  • Familiarize yourself with TriGen and prepare for your volunteer adventure with us!

    Our training session encompasses group introductions, setting care goals, and effectively managing initial interactions with the elderly.

2nd Phase

  • Throughout the cycle, our teams will make regular home visits to check in on their elderly patients.

    During this time, the patient's progress will be closely observed and documented.

    At the Mid-Cycle Multi-Disciplinary Meeting, Team Leaders will have the chance to engage in discussions with a panel of healthcare professionals to enhance the patient's care plan based on their progress.

3rd Phase

  • In the final months of the cycle, teams will re-evaluate the requirements of their elderly.

    Subsequently, they will collaborate with a panel of healthcare professionals during the End-Cycle Multi-Disciplinary Meeting to determine the transition of care for the next cycle or discuss discharge plans.

TriGen@SGH Timeline

1st Phase

  • Get acquainted with TriGen and get ready for your volunteering journey with us!

2nd Phase

  • Home Visits: The teams will engage with your assigned seniors through 1 physical visit and 1 tele-visit every month for a period of 6 months.

    1st and 2nd Multi-Disciplinary Meetings MDM: Team Leaders will present about your patient(s) to a multi-disciplinary panel, while the youths will share ideas about the Youth Innovation Challenge.

3rd Phase

  • 3rd MDM and TriGen Day: Team Leaders will do a final presentation about your patient(s), while the youths will present on the Youth Innovation Challenge. TriGen Day, an appreciation event thanking you for all of your contributions throughout the cycle, will come after this.

Anticipate collaborating with a diverse team of healthcare professionals dedicated to delivering holistic care for seniors. Contribute your professional expertise to enhance their well-being, both in terms of health and social welfare. Additionally, cultivate your mentorship skills by guiding and leading youth volunteers within your team.

Youth Volunteers (Students)

Volunteer alongside Healthcare Professionals and learn about providing Holistic Senior Care. Engage in a Youth Innovation Challenge, pioneering creative solutions to enhance seniors' Quality of Life.
